Reporting Other Conduct of Concern


Use this form to report general complaints against employees or volunteers at Cal Poly Humboldt. These may be directed at individuals, departments and offices, or campus policies. This form may also be used if you are uncertain what other reporting tool should be used. It will be reviewed and forwarded to the appropriate division. 

The University takes allegations of unprofessional conduct, abusive conduct, microaggressions, acts of intolerance, and other disruptive behavior by individuals (whether employees or volunteers) very seriously. It's important that anyone who has experienced such behavior has the opportunity to report it so that the issue can be addressed.

Forms are reviewed during typical business hours. Once submitted, please allow 24-48 hours for processing. If this is an urgent matter, please contact UPD at 707-826-5555. For all emergencies, call 9-1-1.

You are STRONGLY encouraged to provide your name and contact information. This allows us to most effectively address the concerning behavior. While anonymous reports are accepted, they may not be investigated.
